總共操作流程:

1. 創建一個客戶文件夾 (02 John Smith - Humana #2)
2. 複印客戶的紅藍卡 Part A, ID
3. 叫客人攜同 CMS-L564 form 去僱主的人事部簽名, 然後拿回來給我們處理
4. 幫助客人填寫 CMS-40B form B部份添加申請表格
5. 傳真這兩份 CMS-L564 和平 CMS-40B 表格到: 
6. 社安局總局: 833-787-1783 
7. 或者傳真到客戶地區的社安局
8. or apply on ID.Me or Login.gov



Procedure:

1. Create a folder in the client's name (02 John Smith - Humana #2)
2. Make a copy of the client's Medicare Part A card, and ID.
3. Tell the client go to the employer's Human Resources office and ask the manager to fill and sign a CMS-L564 form. Bring it back to us.
4. Help the client to fill out CMS-40B form.
5. Fax both completed CMS-L564 and CMS-40B forms to:

8. or apply on ID.Me or Login.gov
