客戶需要提供以下資訊：

Longer than 6 weeks:

Tell the client it usually takes about 4 to 6 weeks. If they haven't heard back from Social Security after 6 weeks, that means SSA can't verify the client's identity. The client must bring any and all identity proofing documents to the local SSA office. I recommend:

僅新增B部分 Adding Part B only:

1. Create a folder in the client's name (02 John Smith - Humana #2)
2. Make a copy of the client's Medicare Part A card, and ID.
3. Tell the client go to the employer's Human Resources office and ask the manager to fill and sign a CMS-L564 form. Bring it back to us.
4. Help the client to fill out CMS-40B form.
5. Fax both completed CMS-L564 and CMS-40B forms to:
