How to apply
Apply for Illinois Medicaid
Medicaid Qualifications:
2026年白卡低收入申請標準
白卡分高, 中, 低收入3類(QMB, SLMB, QI) (你白卡的信會有註明). 按照個人或和夫婦的每月收入和資產計算去分類:
月收入:
包括退休金, 收租, 工資
資產:
包括銀行存款, 定期存款, 股票, IRA, 共同基金, 人壽保險, 除自住外的其他房產
第1類: QMB (最低)收入人仕:
全包. 白卡會幫支付以下費用:
a. 紅藍卡B部分月費
b. 墊底費(deductible)
c. 共同保險(coinsurance)
d. 所有醫療自付部分(copay)的費用
單身人士資格要求:
月收入低於: $1,355
資產低於: $17,500
夫婦資格要求:
共同月收入低於: $1,829
共同資產低於: $17,500
第2類: SLMB (中度)低收入人仕:

夫婦資格要求:

Medicaid Estate Recovery
If a person owns a house, and has lived in a nursing home and passed, Illinois can go after his house for recovery, even if the house is in a trust.

Tell the client
It usually take about 4 to 6 weeks. If they haven't heard back from Social Security after 6 weeks, that means SSA can't verify the client's identiy. The client must bring any and all identity proofing documents to the local SSA office. I recommend:

Adding Part B only:
- Create a folder in the client's name (02 John Smith - Humana #2)
- Make a copy of the client's Medicare Part A card, and ID.
- Tell the client go to the employer's Human Resources office and ask the manager to fill and sign a CMS-L564 form. Bring it back to us.
- Help the client to fill out CMS-40B form.
- Fax both completed CMS-L564 and CMS-40B forms to:
- Main office: 833-787-1783
- or fax to the office in his zip code
- or apply on ID.Me or Login.gov
